In 2013, there was an unfortunate suicide of a patient in a facility running Kipu. There was an investigation by both the government, and the Joint Commission that cleared the facility of any fault or wrongdoing.
The remediation plan, however, asked: How can the P&P (policies and procedures) be changed so that a possible suicide or other “similar act” could be prevented in the future.
Hence, we invented the “Urgent Issue”.
An “Urgent Issue” can be set up by any staff from a tech or receptionist to a clinical director. For example, if a tech, receptionist, or clinician overhears a direct or indirect comment, they can set up an Urgent Issue like “Mary said she feels suicidal”. From that point, every person who logs in sees the Urgent Issue and your facility’s policies and procedures on “suicide threat” can be followed.
